    If your physician has prescribed emergency medications such as Epinephrine and/or Benadryl, please go to our Health Services Forms page and complete these specific forms:

    • Medication Form for Allergic Reaction

    The completed form and medication should be submitted to the school nurse by the first day of school each year.
